 Care Coordination
     There are three different types of Care  

    Coordination available at the Health

    Center:
      Patient Referral
        Prenatal
        HIV 
 

 Children Health Access Program

   (C.H.A.P)
    C.H.A.P. was developed in 2001 to  

    provide a health assessment, service,

    and education program for children in

    Madison County.

 

Health Disparities Collaborative 

Medicaid Enrollment

 

Patient Assistance Program

    MCCHC is dedicated to helping patients 

    access help with prescriptions. We also 

    have a program called the 340 B 

     Pharmacy Program. 

 

Patient Education Programs
    There are several education   

    programs available to our patients:
      Smoking Cessation Class
        Weight Management (Life-
           Steps) Class
        Diabetic Management Class

         Prenatal Education

 

School-Based Health Clinics

      Wilson's Boys and Girls Club

            Mondays from 3-7pm

        Alexandria School Corporation

            Starting in the fall school year of
