Care Coordination is the process that integrates services according to individual needs.  It is an active, ongoing process of assisting the individual to identify, access, and utilize community resources.  The process includes linking individuals with services, monitoring services received, and developing collaborative arrangements among service providers.  Care Coordination assures that services are delivered in a supportive, acceptable, effective, and efficient manner.
